Assuming this started after an update, but on a PC, I haven’t been able to re-enable it.

It can happen from time to time. If you can reproduce the issue I would recommend to submit a ticket to the support. Meanwhile you can try to apply midpoint constraint manually.
Yes, i have had the same on the iPad! I saved my design, closed Shapr3d, then open it again and it is back. It is since a short while. But I have to admit, I use Shapr3d just a short while in total.
I had it happen, yes.
There is a little “trick” to find the midpoint even if it doesn’t show as a dot for some reason:
If you slide your mouse cursor along the line, the little tool tip changes from “edge” to “intersection” when you’re over the midpoint.
Not ideal, but a kind of workaround when this bug occurs.
